Common forms are provided below. Clients may wish to save time during their first visit by downloading and completing the Client Data and Client Health forms prior to their appointment, though this is not required.
Prenatal clients are required to complete the Client Prenatal Release prior to their first prenatal session. If the client possesses one or more of the conditions listed on this form, a Physician Prenatal Release will also be required prior to the first prenatal session.
